HMO/DMO Insurance Plan - the most restrictive dental insurance option. We do not feel we could participate with HMOs and still deliver the high-quality care our patients have come to expect of us. Patients on these
plans rarely receive the care they deserve.

We recommend that if available, you consider switching to PPO plan. Fee Schedule Insurance Plan - typically union and individually-bought plans. Fee schedule plans provide coverage on a set fee schedule. PPO (PDO) - in most cases, patients with PPO can see their in- and out- of network providers with little difference in their out-of-pocket costs. We accept most PPOs and help our patients by filing their dental claims for them. If you have a PPO insurance, there are a few terms you need to know. Annual Maximum – most PPO plans have yearly maximum of benefits, typically between $1500 and $2500. The insurance company will pay no more than the maximum in any given year. Most plans operate on the calendar year, with new maximums available on January 1. Knowing this information can help you plan your dental care ahead so that your costs are minimized. If you are unsure when your insurance’s year begins, please call us and we will find out this information for you. Deductible – Most plans do not apply the deductible to your preventive visits, such as cleanings and checkups. The deductible amounts may be $25- 100 per person, and apply only to restorative work, such fillings and crowns. Level of Coverage – most PPO plans pay different percentages for different procedures. For example, a cleaning may be covered up to 100%, while a crown may only be covered 40-60%. We try to find out this information for you before we start treatment so that you know what to expect, but frequently insurance companies do not give us exact amounts of their coverage. Usually, before treatment is started, we have at least an estimate of what the insurance will pay, and will always give you this information.
